Q: In the event that I become
sick or injured, what should I do?
A: Report to your Student Health
Center immediately so that proper treatment can
be prescribed or approved. The Student Health Center
may need to stamp your claim form to authorize you
to receive further treatment. Check your brochure
if you are not certain whether this is necessary.

Q: What should I do if my
Student Health Center is closed or I am away from
college?
A: You should immediately consult
a doctor and follow his or her advice. You should
then notify your Student Health Center or us as
soon as possible, no later than 30 days after your
visit.

Q: Is pre-certification
required before I am treated by a doctor or hospital?
A: Certain Plans do require pre-certification.
Please review your plan brochure.

Q: How do I know if my injury
or illness is covered?
A: Please refer to the student insurance
brochure that was given to you upon enrollment.
This brochure will describe those situations that
are and are not covered. If you still have questions,
please contact your Student Health Center or Brown
& Moore.

Q: What if I have questions
regarding my deductibles or premiums?
A: Please refer to the student insurance
brochure that was given to you upon enrollment.
This brochure will detail your obligations for premiums
and deductibles. If you still have questions, please
contact your Student Health Center or the phone
number on your claim form.

Q: What if I pay the bill
for the doctor visit on a covered illness or injury?
How do I get reimbursed?
A: Please print and complete the
online claim form with the appropriate information
and signatures. Attach a copy(ies) of your itemized
bills and/or receipt(s)and send everything to us.
Failure to send in itemized receipts will result
in payment to the care provider directly.

Q: How long will it take
to receive my reimbursement?
A: Please allow at least 14 business
days for your claim to be processed.

Q: If I did not pay for
my doctor visit on a covered illness or injury,
do I still need to fill out a claim form?
A: Yes! Please print and complete
the claim form with the appropriate information
and signatures and mail to the address on the claim
form.

Q: Do I need to fill out
the claim form completely?
A: Yes! Please be sure to fill out
the entire form, including your social security
number, both your college and permanent address,
your date of birth, name of college and college
policy number. Remember to sign the form! Failure
to submit a complete form can result in a delay
in the processing of your claim.
